**Life and Accident Insurance **

This coverage provides group term life insurance.

Payment will be made to your designated beneficiary if you die while the policy is in effect. **Please be sure to designate a beneficiary through the ESC**.

- This coverage also provides group personal accidental insurance (PAI) to those who are eligible. Basic PAI coverage provides benefits for total disability or death occurring within 365 days of a covered accident, or for the accidental loss of a limb or eyesight. (PAI coverage may also be known as Accidental Death and Dismemberment or "AD&D" coverage.) With basic PAI coverage, you are covered on the job or off the job, at home or away from home. 
- There is no cost for basic term life and personal accident insurance (PAI) coverage, since the coverage is employer-paid.
